APPLICANTS WILL BE OFFICIALLY ENROLLED UPON RECEIPT OF A COMPLETED FORM WITH SIGNATURE AND PAYMENT OF FULL REGISTRATION FEES OR COMPLETED PRE-AUTHORIZATION FORM.
